Following the death of two workers in the previous months an incident was reported and determined to require external investigation to maintain independence and to ensure that process was not seen as biased. CSI used recognised incident investigation tools and methodologies. The root causes were identified after interview of 16 personnel over three shifts and it was agreed that the deliberate act that had caused damage, and which was investigated, would not result in dismissal as the individual was in need of support and assistance through the employee assistance program (EAP) which was required by the company as part of legislative compliance.
